Most patients who walk into my clinic have put off their root canal for months — sometimes years. They’ve heard stories. They’ve Googled. They’re convinced it’ll be the worst experience of their life.
As India’s top-ranked endodontist, I see this every week. And every week, the same thing happens: the patient sits down, we complete the procedure, and they look up surprised — “That’s it?”
This guide covers everything I wish every patient knew before walking in — the truth about pain, what the procedure actually involves, why a specialist matters, and why delaying a root canal is the one thing that genuinely does make it worse.
What Is Root Canal Treatment — and Why Does a Tooth Need It?
Inside every tooth is a soft tissue called the pulp — it contains nerves and blood vessels. When the pulp becomes infected (from deep decay, a cracked tooth, or repeated dental procedures), it causes intense pain and can form an abscess that spreads to surrounding bone.
Root canal treatment removes the infected pulp, cleans and shapes the root canals, and seals the tooth to prevent reinfection. The result: your natural tooth is saved, the pain is gone, and the tooth functions normally for the rest of your life.
The alternative — extraction — costs more in the long run (you’ll need an implant or bridge), affects bite and jaw alignment, and is entirely avoidable in most cases.

General Dentist vs. Endodontist: Why It Matters for Your Root Canal
This is the question most patients never think to ask — and it’s the most important one.
Any general dentist can perform a root canal. But an endodontist is a specialist who has completed an additional 3-year post-graduate MDS program focused entirely on root canal procedures and dental pulp diseases. In India, fewer than 3,000 MDS-qualified endodontists exist for a population of 1.4 billion.
The difference shows up in cases that matter:
- Curved and narrow canals — specialists use rotary instruments and CBCT imaging to navigate anatomy that trips up general dentists
- Re-root canals (Re-RCT) — when a previous root canal fails, an endodontist is the right person to salvage the tooth
- Calcified canals — common in older patients; require specialist-level technique to locate and treat
- Complex infections — apical abscesses, internal resorption, cracked tooth syndrome — all need expert diagnosis
Expert Note — Dr. Shilpi Gupta
“About 30% of the re-root canal cases I see could have been saved the first time with proper specialist care. A root canal done right the first time saves you time, money, and a second procedure entirely.”
5 Root Canal Myths — Debunked by an Endodontist
Myth 1: “Root canals are extremely painful”
Fact: Modern root canal treatment causes no more discomfort than getting a filling. Local anaesthesia numbs the area completely. Most patients report the anticipation is far worse than the procedure itself. The pain you feel before treatment? That’s the infection. The root canal ends it.
Myth 2: “Extraction is better — just pull the tooth”
Fact: Your natural tooth is irreplaceable. An extraction creates a gap that causes adjacent teeth to drift, alters your bite, and accelerates bone loss in the jaw. An implant to replace it costs 10–15x more than a root canal. Save the tooth first — always.
Myth 3: “Root canal treatment always takes multiple visits”
Fact: With modern rotary endodontics and digital apex locators, most straightforward root canals are completed in a single 60–90 minute visit. Complex cases with significant infection may require two visits — but this is the exception, not the rule.
Myth 4: “Root canals cause illness”
Fact: This myth originates from a 1920s study that has been thoroughly discredited. Modern research — including studies published in the Journal of Endodontics America, where I have contributed work — confirms there is no link between root canal treatment and systemic disease. Untreated dental infections, however, can spread to the jaw, neck, and in rare cases, cause life-threatening complications.
Myth 5: “Root canal-treated teeth don’t last”
Fact: A properly treated and restored tooth can last a lifetime. At Basil Dental, we back this with a Lifetime Treatment Warranty — because when the procedure is done right by a specialist, we’re confident in the outcome.
Step-by-Step: What Happens During a Root Canal at Basil Dental
Diagnosis & Digital X-ray
Digital X-ray (and CBCT if needed) maps the root anatomy before we begin. Reading canal curvature and detecting accessory canals accurately is where specialist training makes all the difference.
Local Anaesthesia
The area is fully numbed. We use computer-controlled anaesthesia delivery for a more comfortable injection — the brief needle pinch is typically the most discomfort patients experience during the entire procedure.
Pulp Removal & Canal Cleaning
Using rotary NiTi files, the infected pulp is removed and the canals are shaped, cleaned, and disinfected with antimicrobial irrigants. You feel nothing — the tooth is fully numb throughout.
Obturation (Sealing)
The cleaned canals are filled with gutta-percha and sealed with cement. This creates an airtight barrier against reinfection — a critical step that determines long-term success.
Crown Placement
A dental crown is placed over the treated tooth to restore full strength and appearance. We recommend zirconia or PFM crowns — your tooth will look and function identically to a natural tooth.
Signs You Need a Root Canal — Don’t Ignore These
- Persistent toothache — pain that doesn’t resolve with painkillers, or wakes you at night
- Lingering sensitivity — cold or heat sensitivity that stays for 30+ seconds after the stimulus is removed
- Darkening tooth — discolouration of a single tooth often signals internal pulp death
- Swelling or pimple on the gum — a gum abscess (fistula) indicates active spreading infection
- Pain on biting — especially localised to one specific tooth when chewing
- Deep cavity — if decay has reached the pulp, root canal is the only way to save the tooth
⚠ Don’t delay: A dental infection does not resolve on its own. It spreads to surrounding bone and teeth — and in rare cases can cause life-threatening complications. If you have any of the above symptoms, call us today. Same-day emergency slots are available at Basil Dental, open 7 days, 10AM–8PM.
